For Renewal Projects
Intent to Renew
The Intent to Renew form for CoC Renewal Projects was distributed to CoC Recipients and Project Sponsors on November 21, 2025.
CoC Recipients and project sponsors for FY24 CoC grants completed the Intent to Renew in the competition.
2025 Renewal Project Application
The Local Renewal Project application and Application to Transition to Transitional Housing is due no later than noon December 15, 2025 to be considered for inclusion in the local CoC application to HUD. The local applicatoin should include:
- Completed and signed 2025 Renewal Application Form
- Submission of Performance Scoring Form
- HMIS generated APR report for the time period of July 1, 2024- June 30, 2025
- Documentation of non-profit status (IRS 501(c)3)
- Match commitment with sources and uses of matching funds
- Drug-free workplace certification (HUD form 50070)
- Evidence of Submission of Code of Conduct to HUD or inclusion in HUD elibrary.
- Occupancy Agreement or Participation Agreement that demonstrates required participation in Supportive Services.
- Project Services Sheet Indicating services provided by the project sponsor, partner agency, or other agency in the community.
- Attachments indicated in the Renewal Application Form
Projects Requesting Renewal as a Transition Grant to Transitional Housing
- Budget For Transition to Transitional Housing Component (Required for RRH & TH-RRH Projects)
- Brief narrative indicating experience operating a Transitional Housing Program that provides housing assistance and services on a time-limited basis.
*Direct Grantees of CoC funds who are not sub-recipients through UNITY must ALSO complete and submit the application in ESNAPS.
